WS13.01 The cystic fibrosis Home Sputum-Induction Trial (CF-HomeSpIT - ISRCTN86523335) to evaluate home sputum-induction and early morning saliva sampling in children with cystic fibrosis

چکیده
Objectives: In CF patients on modulator therapy, sputum volume and pathogen density is significantly reduced, yet complete eradication uncommon. This mandates a reassessment of current approaches to infection surveillance. Early morning saliva (emSal) home sputum-induction (hSI) may be enriched with lower airway pathogens from undisturbed muco-ciliary clearance overnight present an attractive novel proposition. The CF-Home Sputum Induction Trial (CF-HomeSpIT) non-blinded prospective internally-controlled interventional trial comparing emSal hSI sampling matched same day samples clinic (cSI), saliva(cSal) cough swab(cCS) samples. Methods: CF-HomeSpIT will recruit 100 opened in September 2022. All children at the Children’s Hospital for Wales, aged 5–18 yrs, already using Hypertonic saline (HS) are eligible recruitment. Home packs provided by post consent instructions delivered phone (www.uhwchildren.com/CF-HomeSpIT). For hSI, use HS their normal physiotherapy regimen, SPIT, whenever they feel rattle. Matched taken physiotherapist standard protocols. Results: interim analysis, first 14 identified 6 pathogens. 4/6 (66%) were isolated emSal, 3/6 (50%) 2/6 (33%) cCS, cSal, 5/6 (87%) cSI. combined. A second analysis (with estimated ~40 patients) presented conference. Conclusion: Results suggest home-sampling possible, comparable yield compared supervised sampling. novel, quick, practical approach that warrants further evaluation.

Sputum induction in young cystic fibrosis patients.
A culture from the lower airway secretions is the optimal sample to guide antibiotic therapy in cystic fibrosis (CF) lung disease. The authors therefore examined whether sputum induction is an efficient, safe and acceptable procedure in CF children without spontaneous expectorations. Nineteen patients were studied. Their mean age (range) was 8.6 yrs (4.3-15.2 yrs). Their mean forced expiratory ...
متن کاملSputum induction as a research tool for sampling the airways of subjects with cystic fibrosis.
BACKGROUND Sputum induction (SI) has proved to be a reliable non-invasive tool for sampling inflammatory airway contents in asthma, with distinct advantages over collection of expectorated sputum (ES) and bronchoalveolar lavage (BAL). A study was undertaken to evaluate the safety of SI and to assess if it might be an equally valuable outcome tool in patients with cystic fibrosis (CF). METHODS...
متن کاملUse of the Lung Flute for sputum induction in children with cystic fibrosis: a pilot study.
UNLABELLED This study aimed to assess the effectiveness of the Lung Flute in obtaining a sputum sample from children with cystic fibrosis (CF) that were not productive of sputum with coughing alone. Children attending an outpatient CF clinic who were not able to provide a sample with coughing alone were eligible. Each child used the Lung Flute on two occasions at least one month apart. The prim...
متن کاملMucus, phlegm, and sputum in cystic fibrosis.
Although some clinicians still believe that cystic fibrosis (CF) lung disease is largely due to hypersecretion of very viscous mucus, it has never been demonstrated that there is mucus hypersecretion in CF and it is clear that there is almost no intact mucin (the principal polymeric component of normal mucus) in CF sputum. CF sputum has lower viscosity when compared to asthma or bronchitis sput...
متن کاملSputum itraconazole concentrations in cystic fibrosis patients.
Itraconazole diffusion in sputum was studied in 11 cystic fibrosis patients with allergic bronchopulmonary aspergillosis. There was a high interindividual variability in sputum itraconazole concentration and sputum/serum drug concentration ratio. Three children had sputum drug concentrations before oral administration that were lower than the itraconazole MIC at which 90% of Aspergillus fumigat...
